New baby. Do I need to submit an Actual Delivery Date Document Form?

Actual Delivery Date Document is not needed for the parents who birth the child as long as we have the serious health document and the return to work document filled out by the MD. 

  • It is still needed for the parent that did not birth the child and for adoptions. 
  • The attached forms are needed for the parent that birth the child. 
  • The parent that does not birth the child has to have the MD fill out the serious health form for them. Once the baby is born they must turn in a copy of the hospital verification of birth form for proof of delivery. 

To receive Paid Parental Leave (PPL), the HVOB has to have the requesting parent's name on it. See Paid Parental Leave Policy.


For information about adding your new child to UAB benefits, go to www.uab.edu/hrintouch. Log into the system with your BlazerID. Click on Life Change to add your child to your benefits. This must be completed within 31 days of the birth.
