What happens at the end of my Family and Medical Leave (FMLA)?

Your leave is unique to your situation. Please coordinate with your Supervisor, HR Consultant and HR Leave Team


As an example of one difference,  an employee on FMLA leave for his/her own serious health condition, provides a return to work/medical release form prior to returning to work. The return to work/medical release form is faxed to (205) 975-6900 or emailed to medleavedocs@uabmc.edu.
