No paper documentation is needed to request a leave of absence. The request form is online. You can find it here.
You will give your physician a paper Medical Certification Form. This form must be completed before your leave can be approved.
A verification of birth is needed for the non-birthing parent to be given Paid parental leave.
Before you return to work a medical release document will need to be submitted to Employee Health from your physician, medically clearing you to return to work.
All medical documentation is submitted to Employee Health.
